Please find attached our new “ADHP Member Map”. This project was conceived by our President-Elect, Marie McElderry
and she and I, with the approval of our Board, further developed it as a tool to improve 1) member communication,
collaboration, and spirit of camaraderie, 2) better serve our public, 3) identify geographic areas for expanded
employment opportunities, and 4) help us track critical service types and service-delivery locations data for research
and legislative use. It is important to know that this mechanism was developed as a “Members Only Internal Document”
and not for public dissemination. It is the intention of the ADHP Board to share this information with the Washington
Dental Hygienists’ Association (WDHA) only. If you are in another state, we are currently working state county maps.
Please note which state and county along with the services you provide.
With the support of our Board, I am asking that each member download and save this map. Follow the instructions
below and please report back to me by email by March 30, 2019. Once all data from members is received, it will be
entered on our member roster and disseminated to our active membership and to the WDHA Board only. Please note the
“Practice Type Key” in the upper right and the “County Key” below the map. Here are the instructions for the use of this
1. Identify ALL the types of practices in which you practice. For example: If you work in School based Sealant Program,
Senior Centers, and Skilled Nursing Facilities, then your practice codes would be ACI
2. Identify ALL the counties in which you practice. For example: If you work in School based San Juan, Skagit, and
Snohomish counties, then your county codes would be 28, 29, 31 .

4. If there are ever any changes in your types of practice or practice locations, please let me know ASAP to correct your
entry and keep our data current and accurate.
It is the hope of the ADHP Board that this map will achieve its four purposes and that you will find it useful. Please feel
free to offer ANY input regarding this new tool directly to me at your earliest opportunity. Again, I ask that you return
this vital individual information to me before March 30, 2019, so that we can get this important information up and
running and out there for your benefit.
